A Change in COVID-19 Guidance

Last week, there was a change in guidance to our mask use at Christian Horizons. All of our other Infection Prevention and Control Measures remain in place including Rapid Testing, Active Screening, and the use of masks in specific situations. We will continue to update our guidelines as guidance for our sector changes.

Access to Antiviral 

Early in 2022, Health Canada approved Paxlovid as an antiviral for treating mild to moderate cases of COVID-19. The early drawback to Paxlovid was the limited availability and restricted access.

In Saskatchewan, access to Paxlovid is facilitated by contacting Healthline 811.

In Ontario, with recent updates to who can access Paxlovid as a treatment, people with developmental disabilities now qualify. It is recommended that people we support connect with their primary care provider to see if they would be a candidate for Paxlovid now before they have symptoms or a confirmed case so that a prescription can be available at their pharmacy. There is a quick turn around for accessing Paxlovid, the antiviral must be started in the 5 days after symptoms start. Having a prescription at the pharmacy will help cut down on any delays.

COVID-19 Updates June 16th – June 23rd, 2022

A Change in Mask Guidelines

On Friday June 10th MCCSS released communication outlining expectations around the use of masks in Congregate Care settings. Based on that communication (found here) a number of infection prevention and control measures remain in place including, rapid antigen testing, active screening and the use of N95 during suspected and confirmed outbreaks. The following guidance changes to the universal use of masks at Christian Horizons locations have been made and are now in place:

Mask are NOT required as part of universal (at all times) precautions in Christian Horizons locations for employees, guests, or contractors. Masks will continue to be provided by Christian Horizons for any employees, people supported, visitors and contractors that would like to use them. Masks are required in the following scenarios and additional considerations are outlined below:

 Required (Includes employees, visitors, and contractors)

  1. When there is a confirmed or suspected case of COVID-19 in someone (employee or person supported) who is directly connected to a support location and was at the support location in the 48hrs prior to symptom onset, or a positive COVID-19 test result.
    • Suspected cases of COVID-19 include anyone that would fail active screening
    • Masks remain in place for the home until the illness is confirmed to not be COVID-19
      • For Ontario to confirm an illness is not COVID-19 they require a medical note OR a negative PCR test on day 5 or later OR negative RANT tests on day 6 and 7 OR after 10 days from symptom onset/positive test.
      • For Saskatchewan: A medical note OR 5 days from symptom onset/positive test result.
      • For both Ontario and Saskatchewan, the countdown would restart if new symptoms occur in a different employee or if a person supported becomes ill

2. The person being supported requests the continued use of masks when they are receiving supports where physical distancing can not be maintained.

3.Local District Office Direction

    • The district leadership team may determine a support location has a high risk of negative outcomes due to a COVID-19 outbreak, and require the continued use of masks at all times.

Personal Considerations for Wearing Masks when Providing Care

 When employees are providing direct care, the following should be considered to determine if a mask should be worn. If the answer is yes to any of these considerations, it is recommended that a mask be worn.

                For the Employee

  • Would I prefer/be more comfortable using a mask?
  • Am I likely to encounter droplets (body fluids) while working with this individual?

For the Person Supported

  • Is the person I am supporting medically fragile/complex/immunocompromised?
    • Do they have cardiac (heart) issues
    • Do they have respiratory (lung) issues
    • Are they generally unwell.
  • Is the person I am supporting not vaccinated against COVID-19?
  • Is the person I am supporting elderly (over the age of 55)?

Masks Remain a Requirement

With the upcoming removal of the provincial mask mandates, Christian Horizons continues to follow the Public Health and MCCSS requirements in Ontario. A link to those requirement can be found here. Within the guidance, masks remain a requirement, as does rapid testing for all employees and visitors. In practical terms, this means there will be no changes to our current day to day screening or COVID-19 prevention strategies after Sunday June 11th, when the mandates in Ontario are set to expire. We are seeing similar guidance remain in affect through Ministry of Health overseen organizations such as Long Term Care, Retirement Homes and Hospitals.

We will continue to advocate for updated guidance from the ministry, however due to the recent election and no sitting cabinet at this time it may take some time for updated guidance to be released.

On April 22nd, the Chief Medical Officer of Health for Ontario extended the mask mandate for our sector until June 11th. You can read the release here. At this time all Christian Horizons locations, including those in Saskatchewan, will continue using medical masks except when they can be safely removed as outlined in the PPE FAQ.

Person-supported-COVID-19-Exposure-Flowchart

  • Updated to reflect the same isolation Guidance as the employee flowchart. The guidance is as follows, isolate for 10 days or until a negative PCR test after day 5 or negative rapid test on day 6 and 7.
  • This guidance only applies to Ontario. In Saskatchewan the guidance remains isolate for 5 days.

COVID-19 exposure for employees – Flowchart

  • In Ontario, Public Health released an update that outlines when a PCR test should be done after an exposure to COVID-19. It is recommended that the PCR test be done on day 5. If the employee tests negative on the PCR test on day 5, they can return to work.

  • The self check poster has been updated to better reflect the recommended screening for symptoms and travel by Public Health Canada.

Why the Change in RANT screening?

COVID-19 cases are spiking across Canada.

In Ontario there is an estimated 100,000-120,000 new cases per day.  This is based on waste water levels as other metrics for following infection are no longer reliable due to rules around accessing PCR tests etc. You can view the information from the science advisory table here.

Data is more difficult to follow in Saskatchewan due to decreased reporting around COVID-19 cases and other indicators but a weekly report is published and can be found here. Within the report, hospitalizations and ICU admissions for COVID-19 are climbing. Confirmed cases of COVID in the community appear to be leveling off.

The good news in this scenario is that hospitals in both provinces continue to have capacity to assist anyone who is ill enough that they require hospital based care. The concerning news is that those with developmental disabilities are more likely to require hospitalization and ICU care due to a COVID-19 infection.

It is likely that this wave of COVID-19 will continue through to the middle/end of May based on the current modeling. In Ontario the modeling  suggests similar or greater case counts then we experienced over the Christmas break which lead to significant staffing pressures. With that said, some current signaling as of April 14th is suggesting we may be at the peak of this wave. There is still significant risk for exposure to COVID-19 in our communities across Ontario and Saskatchewan.

With the increased risk of community transmission considered, and an acknowledgement that living with COVID-19, means protecting those that are most likely to suffer negative impact from an infection (the people we serve), we are increasing our rapid testing guidance to every other day.

Congregate living settings serve as homes for residents and a workplace for staff, and protective measures reflect the difference between residents not having a choice on who enters their home to provide service and their own dignity in deciding to take risks in the community.

Access to Anti-virals 

Early in 2022, Health Canada approved Paxlovid as an antiviral for treating mild to moderate cases of COVID-19. The early drawback to Paxlovid was the limited availability and restricted access.

In Saskatchewan, access to Paxlovid is facilitated by contacting Healthline 811.

In Ontario, with recent updates to who can access Paxlovid as a treatment, people with developmental disabilities now qualify. If someone supported tests positive for COVID-19, teams should connect with the persons primary health care provider in order for them to be considered for Paxlovid.
